ABSTRACT

The clinical utility of a life quality score during vincristine chemotherapy of dogs with canine transmissible venereal tumor (CTVT) was investigated using 93 tumor-bearing dogs in this prospective study. At diagnosis, clinical data and the performance status were evaluated according to a modified Karnofsky score (CKS) adapted for dogs. The animals were treated with vincristine sulphate (0.025mg/kg) at weekly intervals until the tumor had macroscopically disappeared. The time until the first adverse event and death were recorded. In a pilot study, CKS revealed a high inter-observer concordance (kappa=0.735; weighted kappa=0.835). CKS permitted a detailed monitoring of adverse effects during therapy. Cox regressions showed that low performance score and reduced body weight were independent predictive factors for death during chemotherapy. The modified Karnofsky performance score is a simple and reproducible clinical instrument, able to estimate patient outcome after treatment of CTVT.


Investigou-se a utilidade clínica de uma escala de qualidade de vida durante o tratamento quimioterápico de cães com tumor venéreo transmissível canino (TVTC) em 93 animais com TVTC que compuseram este estudo prospectivo. Ao diagnóstico, foram avaliados os dados clínicos e o escore de qualidade de vida adaptado para cães (CKS). Os animais foram tratados com sulfato de vincristina (0.025mg/kg) semanalmente até o desaparecimento macroscópico do tumor. Foram anotados o tempo até o aparecimento do primeiro evento adverso e a morte. Em estudo piloto do CKS, alta concordância entre os observadores foi demonstrada (kappa = 0.735; weighted kappa = 0.835). O CKS permitiu um detalhado monitoramento dos eventos adversos durante a terapia. A regressão Cox multivariada demonstrou que o baixo escore de qualidade de vida e o reduzido peso corporal foram fatores preditivos independentes para o óbito durante a quimioterapia. A escala modificada de Karnofsky é um instrumento clínico simples e reproduzível, hábil em estimar os efeitos do tratamento no TVTC.

ABSTRACT

CD95 (Fas/APO-1)-mediated apoptosis plays an important role in immunological regulation and is related to the pathogenesis of autoimmune diseases. Immunoexpression of CD95 has been reported to frequently occur in low grade non-Hodgkin lymphomas, especially of post-germinal center histogenesis, among which those originating in mucosa-associated lymphoid tissue (MALT lymphomas). However, there is no report comparing in situ immunoexpression of this marker in lymphomas and the hyperplastic lymphoid reaction (chronic gastritis) related to Helicobacter pylori infection. The purpose of the present research was to compare the intensity of lymphoid CD95 immunoexpression in 15 cases of H. pylori-related chronic gastritis and 15 gastric MALT lymphomas. CD95 (anti-CD95) was detected by an immunoperoxidase technique in paraffin sections using the catalyzed amplification system. Graduation of reaction intensity (percentage of CD95-positive cells) was semiquantitative, from 1+ to 4+. Nine cases of chronic gastritis were 4+, five 2+ and one 1+. Three lymphomas were 4+, three 3+, four 2+, four 1+, and one was negative. Although 14 of 15 lymphomas were positive for CD95, the intensity of the reaction was significantly weaker compared to that obtained with gastric tissue for patients with gastritis (P = 0.03). The difference in CD95 immunoexpression does not seem to be useful as an isolated criterion in the differential diagnosis between chronic gastritis and MALT lymphomas since there was overlapping of immunostaining patterns. However, it suggests the possibility of a pathogenetic role of this apoptosis-regulating protein in MALT lymphomas.

ABSTRACT

Differences in age and sex distribution as well as FAB (French-American-British classification) types have been reported for acute leukemias in several countries. We studied the demographics and response to treatment of patients with acute myeloid leukemia (AML) and acute lymphoblastic leukemia (ALL) between 1989 and 2000 in Teresina, Piauí, and compared these results with reports from Brazil and other countries. Complete data concerning 345 patients (230 ALL, 115 AML) were reviewed. AML occurred predominantly in adults (77 percent), with a median age of 34 years, similar to that found in the southeast of Brazil but lower than the median age in the United States and Europe (52 years). FAB distribution was similar in children and adults and FAB-M2 was the most common type, as also found in Japan. The high frequency of FAB-M3 described in most Brazilian studies and for Hispanics in the United States was not observed. Overall survival for adults was 40 percent, similar to other studies in Brazil. A high mortality rate was observed during induction. No clinical or hematological parameter influenced survival in the Cox model. ALL presented the characteristic peak of incidence between 2-8 years. Most of the cases were CD10+ pre-B ALL. In 25 percent, abnormal expression of myeloid antigens was observed. Only 10 percent of the patients were older than 30 years. Overall survival was better for children. Age and leukocyte count were independent prognostic factors. These data demonstrate that, although there are regional peculiarities, the application of standardized treatments and good supportive care make it possible to achieve results observed in other countries for the same chemotherapy protocols

ABSTRACT

O mesotelioma maligno de pleura (MM) é tumor de ocorrência rara e nosso meio e tem-se mostrado, quando em elevada incidência, relacionado à exposiçäo pregressa a asbesto ou amianto nas séries descritas em outros países. OBJETIVO. A partir de casos clínicos atendidos no Hospital de Clínicas da UNICAMP, procurou-se evidenciar tal associaçäo epidemiológica, visando alertar profissionais médicos quanto à possível elevaçäo da incidência de tal tumor nesta década e, futuramente, por características próprias da história natural desse tipo de tumor. MÉTODOS. Säo descritos três casos de MM de pleura ocorridos na regiäo de Campinas, SP, num período de dois anos, dos pontos de vista clínico, laboratoriais complementares e anatomopatológico, incluindo minuciosa investigaçäo anamnésica ocupacional e ambiental. RESULTADOS. Todos os casos foram confirmados do ponto de vista anatomopatológico, incluindo microscopia eletrônica, como casos de MM. Os três casos mostraram relaçäo epidemiológica, comprovada por meio de história ocupacional, com exposiçäo a asbesto no passado. Um dos casos teve exposiçäo ocupacional pregressa de curta duraçäo (cerca de um ano); outro teve exposiçäo doméstica a partir de asbesto trazido do ambiente de trabalho por seu pai, durante sua infância, e o terceiro caso com contaminaçäo ocupacional indireta. CONCLUSÃO. A ocorrência de MM de pleura relacionada à exposiçäo a asbesto, no passado, parece ser realidade em nosso meio, como mostram estes três casos clínicos. Destaca-se a necessidade da anamnese ocupacional e ambiental detalhadas na abordagem desses casos, alertando-se para maior atençäo no diagnóstico dos tumores primários da pleura e do peritônio, nos próximos anos, em funçäo de provável aparecimento de novos casos relacionados a exposiçäo a asbesto pregressa, tendo em vista o intervalo de tempo entre o início da manipulaçäo do asbesto em nosso país e o aparecimento destes primeiros casos coincidir com o tempo de latência médio esperado para a ocorrência de MM. Destacam-se, ainda, as necessidades de definiçäo precisa de critérios diagnósticos, para esse tipo de tumor, e criaçäo de registro centralizado de casos.

ABSTRACT

A Terapia Fotodinâmica (PDT) tem-se mostrado uma técnica seletiva promissora no tratamento do câncer. Após injeção intravenosa de um sensibilizador, que é retido por várias horas em células neoplásias, o tecido é irradiado por um laser, e devido a um mecanismo de transferência de energia não radiativa, agentes citotóxicos são produzidos, induzindo as células tumorais à morte. A técnica de hiperoxigenação hiperbárica (HBO) permite elevar o suprimento de oxigênio molecular, nos tecidos tumorais permitindo potencializar a PDT. Para avaliar o efeito combinado (PDT+HBO), desenvolvemos um modelo experimental que consiste na irradiação de tumores sólidos subcutâneos em dorso de ratos.


Photodynamic Therapy (PDT) have been demonstrated a very promising selectivity potential in cancer treatment. After an intravenous infusion of sensitizes, which is retained for severa! hours inside the neoplasic cells, the tissue is irradiated with a laser beam, and due a non-radiactive transfer mechanism, cytotoxic agents are produced, inducing death of the tumor cells. Hyperbaric hyperoxia (HBO) allows the tissues to achieve high supply of molecular oxygen. ln order to evaluate this combined effect of both therapies an experimental model was developed, creating a tumor mass in the dorsal subcutaneous tissue of rats.

ABSTRACT

Case report of a 24 year old female patient with ALL that developed pulmonary invasive aspergillosis during aplastic phase of induction chemotherapy. She was treated with antibiotics and amphotericin B. After recovering from neutropenia, she developed a mycetoma in the inferior lobe of the right lung, which required lobectomy. Nine months after surgery the patient is well, in complete remission of ALL and with no evidence of infection. One month after lobectomy, chemotherapy had been reintroduced. Attention should be called to this form of therapy of Aspergillosis, as a successful way to eradicate this fungal infection that responds poorly to antifungal drugs currently used.
